Advance Serious Illness Planning Workshops

Be Optimally Prepared for Serious Illness with Confidence

Bring together your healthcare, personal care, legal, and financial preparations into one clear, coordinated approach

Workshop Summary

These workshops focus on bringing together all aspects of planning for a serious illness like a heart attack or stroke — helping you clarify your values and preferences for care, while also aligning the healthcare, personal care, legal, and financial pieces that support them. The goal is to ensure your decision-makers are well prepared to speak on your behalf and advocate for you with confidence.

The Advance Serious Illness Plan is a comprehensive, values-based planning process that helps you:

  • Reflect on what matters most to you in the event of serious illness
  • Understand key healthcare and personal care decisions, grounded in real-world clinical experience
  • Clarify how legal and financial planning supports protect your assets and reduce conflict among loved ones
  • Create a clear, coordinated plan to guide decision-makers and care teams

In a supportive, small-group setting, a certified Plan Well Guide Navigator will guide you through thoughtful exercises and practical planning, helping you engage with each part of the Advance Serious Illness Planning process in a clear and supported way.

Workshops are intentionally kept small, with a maximum of 8 participants, to ensure a personalized and meaningful experience.

Who It’s For

The Advance Serious Illness Planning (ASIP) Workshop is ideal for people who want a complete, coordinated plan in place — bringing together healthcare decisions with personal care, legal, and financial considerations. It’s well suited whether you’re just getting started or have already taken some initial planning steps.

This workshop may be a good fit if you:

  • Are new to planning and want clear, step-by-step guidance through the full planning process
  • Already have a named decision-maker or legal documents in place, and want to give them clearer guidance and instructions to follow if they need to act on your behalf
  • Have completed some initial planning and are ready to bring all the pieces together into one clear, integrated plan

Throughout the workshop, each participant works on their own personalized plan.

If you are supporting a loved one and want to help them create a plan, it’s important that you attend the workshop together so they can reflect, learn, and make decisions with your support. Only one registration is required per planner — there is no additional cost for a support person who is attending to assist, rather than creating a plan of their own.

What our Workshop Participants have to say:

★★★★★
My Dad always told my brother and I that we'd know what to do once we saw what state he was in, but that left a lot unanswered. I was able to take the information I learned from the workshop and have a helpful conversation with my father about his wishes. I now know his wishes around ICU care, CPR, and his level of tolerance around possible outcomes including long term care, life support, and mobility challenges. I'm much more confident that I can make decisions and convey his wishes to the medical team.

I personally benefited much from attending the ASIP workshop. It's a bit of a time commitment but the insight I gained and progress I made in planning for my own care was worth it. Even before I complete the legal steps to finalize my plan, I feel good that the preliminary plan in place will lighten my loved ones' burden should they find themselves needing to care for me unexpectedly.

I thought that the guides themselves were great, but added together with the online live navigator, the program is really special. We had the opportunity to ask questions about scenarios and situations. The navigator had really good information and guided us through good conversations.

Upcoming Workshops

Upcoming Advance Serious Illness Planning workshop series dates are listed below. Each session is offered in a small-group format on Zoom to support meaningful discussion and personalized guidance.

Workshop pricing depends on whether you already have access to the Guide to Exploring (the first of the three ASIP guides).

  • If you don’t yet have the Guide to Exploring, your registration includes lifetime access to the three ASIP guides (Exploring, Equipping, and Entrusting), which is reflected in the price.
  • If you already have the Guide to Exploring, we will upgrade your existing access to include the Guides to Equipping and Entrusting.

Both price options include the same live workshop experience and facilitator support — the only difference is whether the Guide to Exploring is included in the cost.

Frequently Asked Questions

Do I need to be tech savvy?

Not at all. You’ll just need to be comfortable using the basic functions of Zoom, such as joining a meeting and turning your camera or microphone on and off. For those doing the step-by-step option, your Navigator will walk you through logging into the Plan Well Guide system and guide you step-by-step through each part of the online Guides. Some participants find it helpful to use two devices—one for Zoom and one for the Plan Well Guide platform—but this is entirely optional.

Do I need to register separately if I’m attending to help a loved one complete their plan?

If you’re attending to support a loved one as they create their plan, we encourage you to attend the workshop together so they can learn, reflect, and make their own decisions with your support.

Only one registration is required per person creating a plan. There is no additional cost for a support person who is attending to assist a loved one rather than create a plan of their own.

What is your cancellation policy?

You can cancel your workshop up to 24 hours in advance without penalty. Cancellations made within 24 hours of the scheduled time are non-refundable.

What if I have to miss a session?

Not to worry. Your Navigator will ensure you don’t miss any of the valuable content. If you’re unable to attend a session, we’ll provide pre-recorded segments so you can review the material at your own pace and stay fully up to date.

What if I already have a Power of Attorney in place?

That’s absolutely fine. Some participants already have legal documents in place when they attend an ASIP workshop.

You’ll still benefit from a deeper understanding of how legal and financial preparations work alongside healthcare and personal care planning, including how to help protect assets and reduce the risk of misuse of funds.

After the workshop, you may choose to review or update your Power of Attorney with your lawyer or notary — or you may decide that your existing document continues to meet your needs. The workshop is designed to support informed decision-making, not to require changes.

I’m not sure if I already have access to the Guide to Exploring. How can I find out?

No problem — we’re happy to help. Simply email us at info@planwellguide.com, and we’ll confirm whether you already have access.
