Welcome to the e-reminder program page!

Normally, it takes 20-30 minutes of your time to complete the serious illness plans that will make it much more likely that you get the medical care that is right for you when you become seriously ill. Future decision-making will also be less stressful for you and your family. 

We imagine you see value to completing your serious illness plan but don’t have time today. That’s why we created an electronic or e-Reminder program to help people who are interested in making their plan, but may need some help staying focused or motivated to see their plan through from start to finish. For some people this can take a few days, a few weeks or a few months.

The point is, we are here to help you and if you are interested in signing up for this e-reminder program then we will send to you a ‘reminder’ by email, encouraging you to complete your plan and finalize the ‘Dear Doctor’ letter (the output of the planwellguide.com planning process).

We will be doing this because the research which shows that people who receive electronic prompts or what we refer to as ‘reminders’ are much more likely to complete what they intended to do compared to those who don’t receive such prompts. These reminders are an important part of this program.

Register for E-Reminders!

Doctors & Medical Professionals
As a health care professional, you are part of team working with patients to provide optimal health care. At Plan Well Guide, we want to help you, so you can further support your patients in a simple and straightforward manner, when it comes to serious illness planning. We created a lot of training materials, resources and tools (such as prescription pads, worksheets, pamphlets, post cards, etc.) to help you implement a Plan Well Guide program in your clinic or setting. These can be found here.

Despite your best efforts to engage your patients in serious illness preparations and planning, many of them do not make the time for it or prioritize it. They may need several ‘nudges’ over time. That’s why we created the “e-Reminder” program.

The benefits of this program ensure you and your team are better able to support patients who you feel would benefit from planning for treatment when they may become seriously ill. The program works as follows.

You begin the program by extending an invitation by email to your patient you want to engage in serious illness planning. If your patient is not used to receiving emails from you, you make precede this e-invite with a face to face conversation or telephone call from your office making them aware of an email from you.

At the bottom of the e-invite email, which explains the e-Reminder program to the patient, they are given a chance to accept or decline enrollment.

When a patient accepts entry into the e-reminder program, they will be entered into the program and automatically begin receiving reminders from Plan Well Guide, which includes the following:

Patient will receive a ‘reminder’ by email every 2 weeks or so, encouraging them to complete their plan and finalize the ‘Dear Doctor’ letter (the output of the planwellguide.com planning process).

Once they have worked through the guide and obtained the summary of their ‘plan’ (referred to as the ‘Dear Doctor’ letter – a sample of this can be viewed, here) they are invited to call your office and make an appointment to discuss the contents of the letter with you, either in person or via telephone.

At this appointment, you need to discuss the contents of the ‘Dear Doctor’ letter so you are able to finalize their serious illness plans together. Reinforcing to them that if they complete this process, they will have the assurance and peace of mind that they now have a plan in the event that they experience a serious illness. Their family will experience less stress if they are called to help make life and death decisions when they are seriously ill.

If the patient does not call your office to make an appointment, Plan Well Guide will also send them regular e-messages to remind them to make the call.

We will send these messages until they have an appointment booked or for up to a maximum of 3 months. A patient is able to cancel these e-messages at any time.

By registering for this e-Reminder program, you will have access to a standardized email which you can share with your patients. This email will have a unique link for your patients, which will allow our team to keep track of which patients decide to opt-into the e-reminder program, which will ensure they receive the various reminder messages, to nudge them along in completing their plan.
