The Guide to Empowering comes in two versions: for the Patient and for the Caregiver

Patient Version

This version is for people experiencing challenges resulting from serious, life-limiting or advanced chronic illness, or the decline associated with aging. Maybe you are frustrated that you can’t quite get the help you need or you feel overwhelmed in managing your illness journey. 

The Guide to Empowering – Patient Version will help identify what needs to be done to better manage your illness journey and how most effectively to do it.

Caregiver Version

The Caregiver version is for people who are caring for a loved one who is dealing with challenges resulting from illness, disability or aging.
